Referrals

Sujatha Sugumaran (Suji) is a physiotherapist, a referral for private health funds is not required. However, patient information and imaging (if any) from GP is always useful.

Care Plans

GP referral is required for treatments under TCA (Chronic Disease Management Care Plan), Motor Vehicle Accident (MVA), Department of Veterans Affairs (DVA), Work Cover and Lymphoedema treatment. However, please be aware that patient care plan does not cover the entire fee.

A gap fee applies on consultations claimed through a Chronic Disease Management Care Plan (not to be used in conjunction with private health funds).

Care and Cure Wellness Clinic has strong links with local GP’s and Specialists with ability to provide referral access to Ultrasounds,
X-Rays and MRI scans.

Department of Veterans Affairs

Clinic accepts patients referred under DVA and a TCA/Chronic Disease Management Care Plan. A gap fee applies on consultations claimed through a Chronic Disease Management Care Plan (not to be used in conjunction with private health) and private health funds.
